UMID system is in compliance with Executive Order No. 420 dated April 13, 2005 which required all goverment agencies and government owned and controlled corporations (GOCCs) to streamline and harmonize their identification system, and Executive order No. 700 dated January 16, 2008 which identified the social security system as the core of the UMID system and directed the SSS to implement the streamlining and harmonization of ID system of all government agencies and GOCCs. source: gsis.gov.ph
I personally applied for the 4-in-1 card (as what they said) by simply dropping by at the SSS branch in Cubao, Quezon City and receive the card after 1 month and a half. Application for the UMID card doesn't have any fee for the first timer.
Here are the steps below:
If you don't have ID's mentioned above you can bring. Valid NBI clearance, School ID, Company ID, Senior Citizen's card, Majot Credit Cards, Voter's ID, Passbook, GSIS membership card, Alien's certificate of registration, TIN card
Note: Participating agencies will do the pictures so you don't need to bring ID pictures.
STEP 2. Go to your nearest participating agencies of UMID card in order to apply or enroll in your area. SSS, GSIS, PhilHealth, Pag-ibig
Since I applied in SSS branch, the following steps below are for the agency policy.
- Secure a form of application for the UMID card ( you can ask the customer service or the information desk to assist you).
- Get a priority number.
- Sign up the form.
- Fall in line or you can just sit and wait for your turn.
- Prepare the needed requirements as mentioned above. (I presented my passport).
- Someone from the SSS employee will call your number, you must be very attentive.
- Once done, wait for another call as someone from the information desk will call you by name and do the photo shot, fingerprint and signature by means of e-signature pad. Wait for the claim stub.
You can receive the UMID card within a month or two after enrolment.
